For a comprehensive behavioral health evaluation and report, simply choose and assessment from the list below! Before you begin each survey, you will have the option to view a sample report and learn more about the assessment. Once you complete your assessment, a PDF report will be generated for you to view and download/print. The price associated with some of these assessments is for this report, which will contain detailed information about your assessment answers. These reports are used and accepted by courts and counselors nationwide.
